If you are a patient and you would like to make a payment, please click on the pay link to the right that corresponded to your invoice location. You will be routed to a secure, HIPAA compliant platform for accepting payments. Acceptable forms of payment include Visa, Mastercard and Discover. Effective 4.1.21, all ForMotion Clinic offices are no longer able to accept cash payments.

REFUND POLICY: If your payment was made in error, please contact us immediately and a refund will be processed using the same payment method used.
